4/5/18 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. The facility failed to ensure chemicals were kept out of reach of residents, potentially affecting 15 residents. A resident in the dementia care unit was observed wandering around the unit before finding a can of aerosol air freshener in her bathroom and spraying an excessive amount in the bathroom twice. The can was found still in the bathroom the next day and displayed warnings typical of household chemicals. Such chemicals in the hands of a dementia resident puts all residents on that floor at risk of dangerous inhalation and irritation.
4/5/18 Failure to provide appropriate foot care. It was reported that the facility failed to identify, assess, or treat thumb nail fungus in a resident. The resident was observed as having inches-long thumb nails that were thickened with discoloration two days in a row and required prescriptions for anti-fungal oral medication and topical cream. Medical notes for the resident failed to note when the fungus first appeared.
4/5/18 Failure to provide care and assistance to perform activities of daily living for any resident who is unable. The facility failed to ensure that five residents who depend on staff assistance for their activities of daily living care received nail care and were shaved. All were observed with long fingernails that had caked brownish-black substances underneath the nail beds. Two had also not been shaved.
