Alden Terrace of McHenry is a large, 316 certified bed, for-profit rehabilitative, “transitional care” facility. Alden’s web advertising describes upscale, “5 star” care, making it appear superior to the standard nursing home. Their website boasts a holistic approach to quality care, using innovative therapies designed to send patients home as quickly and safely as possible.
Luxurious accommodations and indulgent amenities are intended to make Alden a wonderful place to call home while recuperating from surgery or a hospital stay. While many Alden facilities are highly rated by Medicare, and have very few care violations on record, there are always exceptions. Alden Terrace of McHenry is one of those exceptions. With 24 complaints resulting in a health citation in the past three years, this facility was given at two-star “below average” overall rating by Medicare.gov. Here are just a few examples of the care violations:
07/30/2018 Failure to ensure that all necessary precautions are taken to be sure the nursing home environment is free from hazards, failure to ensure safety of all residents from abuse. The facility received multiple citations in relation to an incident where a resident was assaulted and injured by another resident. The resident that attacked has multiple dementia and mood disturbances on their file. They have notes referencing violent behavioral symptoms that is exhibited every 1-3 days.
On the day of the incident, the resident was noted to be very angry and agitated from 3pm and attempted to leave the facility. A CNA was keeping an eye on the resident, but was otherwise passing out medicines and attending to others. When she checked in, she saw the resident pull another resident by the hair, and they landed on the floor, bleeding. The attacking resident should have been removed from the area before it escalated to someone getting injured.
06/11/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. Pressure injury reduction measures were not taken for multiple residents that are at high risk for developing pressure sores. One resident was observed with no pressure reduction measures in place, and while they were supposed to be repositioned every 2 hours to reduce pressure, they were observed to have not been turned in almost 4 hours and care staff was not clear on what sort of other pressure interventions should be in place.
06/11/2018 Failure to provide enough nursing staff every day to meet the needs of every resident; and have a licensed nurse in charge on each shift. Based on observation, the facility failed to provide sufficient staffing to meet the residents’ needs. Care like toileting and changing of incontinence briefs that should be happening every two hours is only happening a few times a day, sometimes leaving individuals in soiled and soaked incontinence briefs for 10 hours. Some residents stated they are not showered regularly.
