Alden Estates of Naperville is a 200-bed for-profit rehabilitative, “transitional care,” and short-term health care facility. Alden’s advertising paints a picture of upscale, “5 star” care, something beyond your standard nursing home. Their website boasts a holistic approach to quality care, using innovative therapies designed to send patients home as quickly and safely as possible.

Luxurious accommodations and indulgent amenities are intended to make Alden a wonderful an enjoyable place to call home while recuperating from surgery or a hospital stay. While many Alden facilities are highly rated by Medicare, and have very few care violations on record, there are always exceptions. Alden Estates of Naperville is one of them, with 19 complaints resulting in a citation in the past three years, and a below average rating with Medicare.gov. Here’s some examples of the violations seen:

08/10/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. Fall prevention measures were not implemented, resulting in multiple injuries. One individual fell 11 times, one of the times resulting in a deep laceration to the bridge of his nose, requiring an emergency room visit. Another fall resulted in another emergency room visit and 3 staples to the back of the head. His care plan still did not reflect the need for extensive assistance to prevent falls. Under this same citation, there were two other notes of improper transfers that were observed with other residents, which could have ended catastrophically.

7/20/2018 Failure to provide necessary care to maintain physical and mental well-being, failure to uphold Nursing Home Care Act – shall not abuse or neglect residents. The facility failed to assess the progression of skin wounds, failed to develop a care plan for said wounds, and failed to consistently care for pressure wounds, resulting in painful, necrotic pressure injuries in multiple residents.

In one instance, the family complained of inattentive staff that did not properly move their loved one, even when they call on them to come, allowing him to develop a large pressure sore. The surveyor noted that upon observation, this resident was not repositioned from 8am – noon, when the surveyor then called upon a wound doctor to check on the resident. It was then recommended that the resident go to the hospital for surgery on the wound.

Another resident was observed (at 10:40am) with an empty feeding pump beeping with an alert. Multiple staff members were observed passing by the room, but no one responded to the alarm. The alarm continued until a CNA was alerted, at 1:00pm.

This resident also had a pressure wound that had deteriorated while in care at the facility. He had also been sent to the hospital for surgical debridement. The wound doctor noted that in order for the wound to heal properly, he should not be positioned on his back, and should be repositioned regularly.

Additional wounds were observed, with no documentation on file, no treatment order carried out. The nurse assessed the wound on request and described the wound as – 15cm X 14cm and 4.5cm deep, with muscle exposed.

Yet another resident was observed to have a pressure wound care plan that indicated keeping her clean and dry, repositioning her every two hours. At the time of observation, this individual had been sitting in a soiled incontinence brief for over 4.5 hours.
