11/9/2023 Failure to provide enough food/fluids to maintain a resident’s health. The facility failed to ensure that a resident identified as having significant weight loss received his gastronomy tube feeding as ordered by the physician to prevent further weight loss.

11/9/2023 Failure to provide for the safe, appropriate administration of IV fluids for a resident when needed. The facility failed to change the resident’s midline dressing to ensure integrity of the catheter and to prevent potential intravenous site infections. The facility also failed to document the midline site/location, as well as the baseline external midline catheter length and mid-arm circumference per policy and procedure.

11/9/2023 Failure to provide safe and appropriate respiratory care for a resident when needed. The facility failed to ensure residents received oxygen therapy as ordered by the physician during multiple observations.

11/9/2023 Failure to ensure medication error rates are not 5 percent or greater. The facility failed to ensure medications were administered according to the physician’s orders. There were 2 medication errors out of 25 opportunities, resulting in an 8% medication error rate.

 11/9/2023 Failure to provide and implement an infection prevention and control program. The facility failed to implement infection prevention control measures and follow facility policy related to hand hygiene while providing care and during medication administration.

11/9/2023 Failure to develop and implement policies and procedures for flu and pneumonia vaccinations. The facility failed to offer the influenza and pneumonia vaccines. In a sample of 33, it was found that 5 out of 5 residents who were reviewed for influenza or pneumonia vaccines had not been offered them.
