Alden Meadow Park is a short-term rehabilitation and health care facility that offers residents transitional care and skilled nursing in luxury accommodations. The facility has 94 beds and is Medicare-certified.

Medicare awards Alden Meadow Park an overall rating of Average. This rating includes an impressive Above Average rating for both quality measures and health inspections, but Much Below Average for staffing. Below are a few of the infractions recorded in recent health inspection and complaint inspection reports:

12/18/2023 Failure to ensure that residents reviewed for Activities of Daily Living received necessary services to maintain good nutrition grooming. A resident voiced concern that staff were not brushing his teeth, when he was unable to do so. This resident was admitted with a care plan that included assistance with personal care. However, 10 days after this care plan was put in place, the resident complained that his teeth had not been brushed since he had arrived. The resident’s teeth were noted as rough with thick white material along the gum lines.

12/18/2023 Failure to ensure that a resident who is fed by enteral means receives the appropriate treatment and services. Facility staff did not check the placement of a resident’s gastrostomy tube by residual prior to medication administration. When asked, a staff member indicated staff at the facility do not check placement before administration, and they check by x-ray. When reported to the Director of Nursing, she said the residual should have been checked prior to medication administration.

7/27/2023 Failure to invite or hold care conferences for two residents after admission. The nursing home administration stated, “I admit this is a project” and indicated that a care conference for each resident should be held within 48 hours after admission, a two-week check-in, and quarterly for long-term residents.

7/27/2023 Failure to document the grievances of 3 residents, conduct thorough investigations of the issues identified by the residents, or provide resolution to concerns brought to the attention of staff. One resident voiced concerns with facility staff regarding pest issues and missing items. Another resident voiced concerns with facility staff regarding missing clothing. The third resident voiced concerns regarding pest issues. The facility did not file a grievance for any of these concerns and failed to follow the facility grievance policy.

7/27/2023 Failure to ensure effective pest control for 3 of 3 wings, having the potential to affect 66 residents. Residents voiced concerns of being bitten and awakened by pests and having pests, including flies and beetles, crawling on them. The surveyor observed a variety of pests including flies, wasps, and moths throughout the building. Spider webs were also found throughout the building.

The key is to pick a nursing home with good ratings. These ratings often come from government programs like Medicare. You can find such ratings on medicare.gov or other government websites. You might also want to research reviews for local nursing homes shared online by current or previous residents and their families.

When you find some highly rated facilities you’re interested in, schedule tours so you can see the properties themselves. On tours, pay attention to how clean the facility is, what services and amenities are offered, and how the staff interact with residents. Ask questions about the vetting of staff and facility protocols. If possible, visit again on a non-tour day to see how the facility functions when it’s not expecting guests.

After you select a nursing home, stay in close contact with your relative so you can monitor their care and they can report any mistreatment to you. It’s helpful to visit regularly so you can see how they are for yourself. Of course, you don’t want to worry unnecessarily, but do be on the lookout for any signs of neglect or abuse that your loved one can’t or won’t tell you about. These signs can be many, but here’s a few to consider:

  • Untreated severe bedsores
  • Unexplained injuries like bruises, burns, or scars
  • Evidence of poor circulation like sunken cheeks or eyes
  • Weight loss or malnourishment not due directly to an illness
  • Frequent trips to the emergency room or doctor
  • Wrong type or the wrong amount of medication administered
  • Poor hygiene or unwashed clothes or hair
  • Instances of wandering or elopement
  • Frequently misplaced or lost personal items
  • A lack of basic necessities, including utilities, water, and food
  • A dirty room or bathroom
  • Frequent trouble sleeping
  • Unusual violent or withdrawn behavior
  • Sudden depression or confusion
  • Signs of trauma like rocking back and forth

Taking Action

If you have reason to believe a loved one is suffering abuse or neglect at the hands of nursing home staff, take immediate action to prevent it from continuing.

Begin documenting any incidents or indications of abuse. Take your concerns and documentation to the facility’s management and explain what you know and suspect. They should be willing to hear you out and investigate the situation. If what you’ve said is found to be true, they should take immediate action to rectify the situation. However, if they react to your statements dismissively or defensively, or fail to take satisfactory action, you may need to seek legal advice.
