Alden’s advertising paints a picture of upscale care, staging themselves as offering care and amenities beyond the standard nursing home. Their website boasts a holistic approach to quality care, using innovative therapies designed to send patients home as quickly and safely as possible.

Luxurious accommodations and indulgent amenities are intended to make Alden Park Strathmoor an enjoyable place to call home while recuperating from a hospital stay. 08/02/2018 Failure to take measures to prevent falls, taking precautions to assure that the resident’s environment is free from hazards, failure to notify physician of any accident or change in condition. Multiple failures in care led to a resident developing an acute subdural hematoma and small subdural hemorrhage. The resident, who was in the facility following a cerebral infarction (similar to a stroke) and has one-sided paralysis as a result, fell while attempting to reposition herself in her wheelchair at dinner.

She flipped herself over the side armrest, landing on her head. She had notes on file that she was a high fall risk due to her restless behavior in the wheelchair, evidenced by her moving her legs in a way that changes her position in her chair.

The fall occurred at dinner time, her condition rapidly deteriorated (became lethargic and vomiting) and due to a lack of following proper communication protocols, she didn’t leave via ambulance until after 11:15pm.

03/19/2018 Failure to immediately tell the resident, the resident’s doctor, and a family member of situations (injury/decline/room, etc.) that affect the resident. A resident fell while in the bathroom, and their doctor was not informed. The residents’ file had a note that was dated 9 days later (and the same day of the complaint investigation) stating that the physician was contacted.

05/04/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. Several residents were observed to have pressure sores. Based on observation, the facility failed to take precautions to prevent these sores and failed to provide pressure relieving measures as needed.

One residents’ family members report that the resident shouldn’t be on his back, but is regularly, and is rarely repositioned. Another resident’s skin notes showed documentation of wounds that didn’t align with what was present on the body, some notes saying stage 2 appearing with no previous indication of breakdown, some saying stage 3.

The director of nursing was noted to have said that they doubt the assessments were performed accurately. Yet another resident was observed to have a pressure ulcer, but was observed to have no measures in place to reduce pressure.
