1/31/2019 Failure to develop and implement policies and procedures to prevent abuse, neglect, and theft. The facility failed to recognize and investigate an allegation of abuse. A resident complained that a CNA threatened to take her life, and the facility managed the situation by having the CNA apologize and not care for the resident again. No investigation was done into the verbal abuse, however.

1/31/2019 Failure to ensure that a nursing home area is free from accident hazards and provide adequate supervision to prevent accidents. The facility failed to investigate a fall and put interventions in place for two residents. A resident tried to get out of bed and fell, sustaining no injuries. It was discovered that the resident had fallen before but not progressive intervention was added to his care plan to prevent future falls. A second resident was found sitting on the floor after sliding off her bed while trying to get up. Despite instructions in her care plan to keep her bed at a height that allows her to put her feet flat on the floor, her bed was still too high.

1/31/2019 Failure to provide enough food/fluids to maintain a resident’s health. Staff failed to provide nutritional supplements and adequate fluids to two residents. One resident was at risk for nutritional problems and was to be served supplements per order. During two observations her meal went uneaten with no encouragement from staff and no supplement was offered. Documents show her weight had dropped 15 pounds in four months. A second resident was observed with a dry mouth and no water readily available. Staff did not offer fluids during toileting despite the resident’s care plan stating she is at risk for dehydration.
